We are excited that you have chosen us to be your child’s pediatric dental provider. Our Mission is to treat each child and parent with respect patience and the highest standard of care. We promise to share our knowledge and enthusiasm with each child so they may appreciate the importance of preventative care, and experience a lifetime of healthy smiles.
The First Visit
Our goal is to develop a trusting, and healthy relationship with your child. This is accomplished by involving them in the appointment. The first visit is to introduce your child to our staff, the office and the dental environment. We will evaluate and document your child’s dental and medical history, perform an oral cancer screening and discuss any cavities, missing or unerupted teeth. We will determine the relationship of their bite and review any oral habits that exist. We will formulate an oral hygiene program with instructions that are pertinent and specific to your child’s age and needs. We will also answer any questions or
concerns that you may have regarding your child and their dental health. Remind your child that routine dental visits are going to be a normal part of growing up, and that you ALSO visit the dentist. Explain that we will count their teeth and make sure they are healthy. We recommend that children be seen for
preventative care every six months, just like adults. By developing a positive relationship with your child early in life, we can help PREVENT decay, dental disease and improve total body health.
We ask that you bring your completed forms with you to your child’s visit. This will allow us to spend the entire appointment focusing on your child. Since our younger patients tend to be at their best earlier in the day, we prefer to see all children under the age of four in the morning hours. We will do our best to accommodate treatment requiring a shorter appointment in the after-school hours. Please remember that the after-school period is usually the first to be selected. It may be necessary to schedule your child in the morning or midday to keep treatment on a timely basis. Also, consider alternating “prime” appointment times so all parents may have that opportunity.
We acknowledge, your busy schedule and pride ourselves on seeing our patients on time. You can help us by arriving a few minutes early for your child’s visit. Our appointments are short in duration (20 to 40 minutes) and arriving even a few minutes late may not allow us to complete the scheduled treatment. 24 hour notification is appreciated if it becomes necessary to reschedule. In the event of an emergency after our scheduled hours, please call the office for Dr. Margaret Miller’s emergency contact number.
We have enclosed a copy of our financial agreement.
For Patients With Insurance: An insurance information form is also enclosed. Keep in mind that our records will only be as accurate as the information YOU provide. With today’s technology, our claims are forwarded almost immediately after treatment is complete. If the information form is incomplete, we will be unable-to submit a claim and payment will be required at the time of service.
For Patients Without Insurance: Payment for professional services are due at the time of the appointment. Should there be an extensive treatment plan, special arrangements can be made.
